St Clare's day centre provides the focal point for our referrals procedure as well as an initial contact. Our team consists of Community Link Team manager, Mental Health Worker, Substance Misuse Worker, Resettlement Worker, Homelessness Prevention Worker and Project Workers. Agencies can phone or fax the centre to obtain a referral form on:

Please bear in mind that if you are referring a client to us then we will need to have these forms fully completed along with a risk assessment of your client for consideration. Waiting times to see referrals will depend on the current demands upon the service.
